Use this form to request training and technical assistance from the National Juvenile Information Sharing Initiative — NJISI. NJISI assistance includes: requesting conference speakers, assessing your collaborative juvenile information sharing issues, implementing the OJJDP NJISI model, training on the National Information Exchange Model (NIEM) for technology staff/team members and for Executives /Managers (Executive Briefing), and developing information sharing strategies, assistance with the development of governance structures, communication plans and, development of policies and procedures on privacy and consent practices and assistance with developing measurable outcomes and performance measures for your juvenile information sharing initiative. All fields, except for "fax," are required.

1. Problem Statement: Describe, as specifically as possible, the condition or issue for which the training or technical assistance is requested. If the requesting agency participated in the NJISI Readiness Assessment process, please relate the problem back to the issues raised on the survey

2. Previous Efforts: Have there been any previous attempts to address the condition or issue for which the training or technical assistance is requested? If so, what action(s) were taken and what were the results?

3. Training or Technical Assistance: Describe what type of training or technical assistance you would like to receive. If the requesting agency participated in the NJISI Readiness Assessment process, please relate the problem back to the issues raised on the survey.

4. Target Audience/Recipients: List the intended audience or recipients of the training or technical assistance.

5. Goals and Outcomes: Describe the goals and outcomes you would like to achieve as a result of this training or technical assistance. Include how this will assist you in achieving the goals of specific purpose areas indicated above.
